Dental Savings Plans

Any patient who does not have dental insurance can purchase a membership. For new patients to our practice over 18 years old, there is a one-time $75 new patient setup fee. Memberships are not considered insurance. All dental care must be provided by Kaukauna Family Dentistry and does not include referral care. Membership benefits are not transferable to another person or another dental office. It is the member’s responsibility to schedule and keep all appointments for included services. The membership is valid for one year from the start date, and all benefits must be used within that period. Unused services cannot be carried over to the next membership year. Membership payments may be made by cash, check, or credit card. Treatment must be paid in full by cash or check on the day of service to receive the 15% discount, or by credit card for a 12% discount. CareCredit cannot be used to purchase membership plans but may be used for additional treatment without membership discounts. Membership payments are non-refundable. Kaukauna Family Dentistry reserves the right to update the terms of our membership plans, with changes applying only at renewal while honoring all active memberships.
